“PAWS & BROWN” is the leading provider of health and life insurance for your GT Bear. We deliver strong
customer service and provide easy-to-understand information. The result is a wide range of health and life
benefit options for your bear that insure a swift recovery. You will only be responsible for the $5.00 co-
payment.

GT BEAR EMERGENCY HOSPITAL is devoted to meeting your bear’s medical needs, from basic first aide to
major surgeries. At GT BEAR EMERGENCY HOSPITAL, our trained staff will consult and make the best
decision for the well-being of your bear. If your bear is beyond recovery; we will replace it at no charge, just
the $5.00 co-payment.

Since all of the physicians at GT BEAR EMERGENCY HOSPITAL participate in “PAWS & BROWN” health
insurance plan, there is never an additional cost to you for the doctor’s services.

At this time our insurance provider, “PAWS & BROWN”, only allows that we offer medical treatment to bears
born at GT BEAR EMERGENCY HOSPITAL.

We at GT Bear Emergency Hospital will support you in aiding your bear’s swift recovery. To do so we must
have some basics information. Print the form and fill it out completely.

Please do NOT send any outfits, or accessories with your bear, all of his/her needs are taken care of at the
Hospital. (If anything is sent, we do not guarantee its return).

Send him/her back to the GT Bear Emergency Hospital and we’ll get him/her back to you as soon as
possible.

Method of Payment: Through the website of Greenwich Teddy Bear (gtbear.
com) we accept major credit cards including PayPal. We do not accept money
orders or checks – this is due to the length of time it requires to get checks
approved. At this time we do not accept orders or payment by mail.

Shipping: Orders are shipped free within the USA. Shipping is through USPS
priority. USPS priority usually is delivered with in 2-3 days according to the
postal service. Most orders if in-stock, will ship out of our warehouse within 24
hours.

Sales Tax: All CT. customer orders are changed a 6% state sales tax.

International Orders: International orders are not accepted at this time.

Returns: Most orders, unless otherwise specified, can be returned or
exchanged within 7 days of receiving. All Returns must be in like new condition
and still in its original packaging. Special orders such as imprints and
embroidery can not be returned. Customers are responsible for return
shipping and all approved returns will be reimbursed on receipt.

Return Procedure: Al returns must receive a return number. Return numbers
can be obtained through a phone call or an e-mail communication. Any returns
should be sent back to Greenwich Teddy Bear as stated on the shipping label
and packaging slip. Once we receive your return, a refund will be issued back
to the credit card used for the purchase. When returning an item to Greenwich
Teddy Bear be sure to include a note with your order number and the details
for the return.
